Request your Immunization Record if you have lost your COVID-19 vaccine card

COVID-19 Vaccine Records Process (For Pennsylvania only)

  1. Visit this website: Philadelphia Immunization Record Request Form

Option 1:

*NOTE: Website is only available in English and Spanish

  1. Fill out form and upload a copy of a valid state ID, driver’s license, or passport
  2. Select whether to receive records via email or mail
  3. Submit form online, wait a few days to receive the records. If by mail, it will take longer.

Option 2:

  1. Download and print out Philadelphia Immunization Record Request Form
  2. Fill out form
  3. Scan and print out a copy of a valid state ID, driver’s license, or passport
  4. Submit the form and copy of ID
    1. MAIL documents to PhilaVax (address is on the form)
    2. SCAN and EMAIL the documents to PhilaVax@phila.gov or covid@phila.gov
      1. *NOTE: Please pay attention to the email provided on the form you are using and send it to that email address

*NOTE: Available in English, Spanish, Simplified Chinese, Vietnamese, and Korean. This option will take longer, but they do have forms translated into multiple languages and it doesn’t require using a computer if being mailed.

Love Your Family, Get Vaccinated! 愛你的家人，就接種疫苗 ! Video Campaign

Philadelphia Chinatown Development Corporation presents our “Love your family, get vaccinated! 愛你的家人，就接種疫苗！” campaign in encouraging our community to get vaccinated. In this episode, our friend Momo learns how to overcome his stigma towards the COVID-19 vaccine through his love for his family.
